Neighborhood HealthSource is a non-profit, multi-location, full service primary care clinic organized as a Federally Qualified Health Center. Watch this video to see what sets us apart.

Our clinics are staffed by highly skilled Doctors, Certified Nurse Practitioners, Physician Assistants, Certified Nurse-Midwives and Registered Nurses. Our family physicians complete specialty training and are board certified. Our Nurse Practitioners and Nurse-Midwives complete advanced training and are certified in primary health care. Neighborhood HealthSource is a Federal Tort Claims Act (FTCA) deemed facility. 

This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

We are proud to be a member of FUHN (Federally Qualified Urban Health Network), a coalition of 10 Federally Qualified Healthcare Centers (FQHC) at the forefront health care reform with the initiation of one of the nation’s first safety net provider Accountable Care Organizations (ACO). Research shows that Medicaid and uninsured patients typically have the highest Emergency Room utilization rates. FUHN is participating in a Minnesota Department of Human Services Medicaid project with the aim of delivering higher quality health care, reducing the total cost of care, and improving patient access and patient satisfaction through innovative approaches to care and payment. Collectively, the 10 FUHN clinics include 33,000 Medicaid patients in this project, 10% of whom use Neighborhood HealthSource for their care.

Over 3 years, FUHN achieved a 8% decrease in inpatient admissions, 18% reduction in emergency room utilization, and showed patient satisfaction over 90% on three key metrics. FUHN has been recognized as one of the nation's leading ACOs by numerous health care reform researchers and thought leaders.
